How long is the program?

We ask that participants be willing to make an initial committment to our Phase I- Physical Wellness Program for 60 days. Upon completion of Phase I, participants will have an opportunity to apply to additional phases to move through the full 12 month program. It is not a requirement to complete the full 12 months. The phases are broken down as follows.

Phase I- Physical Wellness

2 months

Phase II- Spritual Wellness

4 months

Phase III- Psychological Wellness

4-6 months

Upon graduation from the full 12 month program residents have an opportunity to apply to the Discipleship Program, our independent transitional living program.

Do we take insurance?

We do not accept insurance at this time. We are a self-pay program. We are happy to provide you with the information needed to seek reimbursement from your insurance company for our services.

What is the cost of the program?

The cost of the program is $30,000 a year. Due to the giving of our generous donors, Transformations is able to offer an 80% discount to individuals in need of a program scholarship.

We require participants to pay a minimum of $600 a month. If eligible, these payments can be deferred until participants begin employment.

An initial $600 is requried at the time of intake.

We work closely with individuals and families to provide several payment plan options according to the needs of the resident. Please contact us for an assessment to review these options.

How do I set up an assessment?

Please fill out the residential interest form or call 317-219-6673 to schedule an assessment.

What should I bring to the assessment?

Please bring a photo ID, insurance card, any court ordered agreements (if have pending legal charges or on probation) and any contact information for Releases of Information for anyone you approve us to be in contact with about your care, including: primary care doctor, probation officer, attorney, therapist, family members or others who will be involved in your recovery care plan. Please bring your personal items in 1 suitcase.

Following is the allowable and recommended item list:

  • 14 comfortable outfits
  • 7 workout outfits
  • 2 casual/dress outfits
  • Pajamas
  • 2 swimsuits (No bikinis)
  • 10 pair of underwear
  • 10 pair of socks
  • 4 pairs of shoes (1 flip flop, gym shoes, work shoes, 1 other)
  • 1 winter coat and 1 jacket
  • 1 hat and gloves
  • Laundry Detergent
  • Unopened over the counter pain relievers (except for cough suppressants)
  • Stamps and envelopes
  • Hygiene Products (without alcohol)
  • Blow-dryer, curling or straightening iron
  • Fingernail polish, emery board, nail clippers, tweezers
  • Eyeglasses, contacts and accessories
  • Appropriate family photographs
  • Chewing Gum
  • Robe and Slippers
  • Make-up
  • Battery operated alarm clock
  • Small fan
  • Planner
  • 1 Pillow and 1 blanket (optional, as bedding is provided)

Non-allowable Items (includes, without limitation):

  • Unapproved prescription medication
  • Drugs, drug paraphernalia, or cigarettes/nicotine products
  • Clothing w/ inappropriate messages or sexually explicit/revealing clothing
  • No revealing clothing, shorts shorter than 4”, shirts that show cleavage or bellys, see through leggings, etc
  • Weapons or gang related materials
  • Televisions
  • Candles/Incense, lighters
  • Cell phones or electronics
    • Cell phones will be held in client property if brought upon intake

You will be allowed to exchange clothing or accept new items. Item drop offs must be done by approved contacts with pre-approval from staff. Visits are not permitted when dropping off items unless scheduled.

What is the policy on tobacco use?

We are a tobacco-free and nicotine-free facility.

Do we offer medical detoxification services?

Transformations Center for Healing is not a medical facility and does not have medical personnel on staff. Therefore, we have strict rules regarding admitting individuals in active withdrawal. Due to the medical risk associated with alcohol or benzodiazepines, individuals must be medically detoxed from these substances at a medical facility prior to being admitted to Transformations. It is required that residents must present a medical release stating that they have completed medical detoxification and/or are not currently in active withdrawal from these substances.

Admitting individuals for all other substances is left to the discretion of Transformations’ staff based on the outcome of the intake assessment.

Can I bring my prescription or non-prescription drugs?

Due to the holistic, non-medication structure of Transformations’ program, residents are not permitted to take any psychotropic or narcotic medications at any time. The following is a list of approved medications.

  • Pain Reliever - Tylenol, Naproxen, Ibuprofen, Excedrin
  • Stomach Medication - Prilosec OTC, Nexium, Prevacid, Tums, Rolaids, Pepto Bismol
  • Cold, Cough or Sinus Medication - Claritin, Chlortabs, Tylenol Cold & Sinus (all non- drowsy), Alka Seltzer Plus Cold Daytime, Mucinex, Alcohol Free Cough Syrup, Tussin CF, Cough Drops (Cannot take antihistamines or decongestants ending in “D” or “DM”)
  • Other: TCFH vitamin regimen, probiotics
  • Prescription antibiotics
  • All other vitamins and prescription medications must be deemed medically necessary by a medical doctor in order to be eligible for approval
